Oximetry @ Home A virtual ward remote monitoring solution
Health Call Oximetry @ Home was produced in collaboration with front line teams at South Tees Hospitals NHS Foundation Trust and Tees Valley Clinical Commissioning Group in response to the COVID-19 pandemic.
The Oximetry @ Home pathway, was initially developed as a ‘step down’ from secondary care, but can also be used as a ‘step up’ from primary care.
Eligible patients can be discharged from hospital earlier and reunited with their family or carers, whilst being closely monitored by clinical teams. Likewise, patients who have received a COVID-19 diagnosis can be monitored by their primary care teams. This means they can spot deterioration early so that the patient can be cared for appropriately.
Being able to discharge patients earlier means that spaces are freed up at a time when the NHS is under significant pressure. It also means that those most in need of intervention can be identified quickly and lead to better outcomes for them.
How does Oximetry @ home work?
- Eligible patients are added to the system and their bespoke oxygen levels are set by their clinician
- Twice a day, patients receive a text messages asking them to provide their breathing rate, heart rate, oxygen saturation, temperature (if they have a thermometer), type of oxygen supply and other details which may indicate deterioration
- Once completed, patients receive tailored advice
- Clinical teams receive a colour coded response based on the patient’s readings in the clinical portal
- Automatically issued digital survey
- Patient use the mobile app when they access the survey
- Text message reminders
- Background triage logic to determine colour coding of submissions
- Web-based management portal where survey submissions are accessed
